I had several "since never" answers too but just left those unchecked altogether. Seemed to work.

You should have your instructor look at your survey; I can't imagine how those questions and answers will provide any usable information.

 

Your instructor can guide you to a survey methodology that will net relevant information.

Maybe it's not about the survey data. Maybe the students are going to analyze snarky and crabby forum responses to a questionable survey and gather data about forum dynamics How meta.

Ha ha ha... as a third year university student having taken stats, psych and biology, this is the type of stuff you have to do ALL THE TIME for the first 3 years. Boring, but if it gets you an A, good job.
