EDIT: I didn't see any image. It was there when I checked a few hours ago when this thread was young, but it seems to have been removed since then. If some sort of system was implemented in recent months to automatically strip out HTML code from the name, I think it would only do so when the TB was edited, therefore older TB's would retain the image code if they're never edited. This may be the case here.
