There is one premade full-sheet logbook in Word format (132 Logs Doc) on that page, or here. You can print it as is, edit it to your liking, or you can just use Word to make your own. I experiemented with cutting up some paper and stapling it together, however I think it's more useful to print a full sized page with room to leave the name, date and comments.
