I've seen this happen when someone goofs up logging from a phone. The one case I know about personally, the system was telling them that their log was rejected. They kept trying again for each additional find they tried to log, getting rejected each time. It turned out that some of the logs were accepted, just not all of them, so every time they retried, duplicates were created. I think this turned out to be a bug in the phone software that was eventually fixed, but perhaps this person has old software.
