First of all, since your profile lists only 1 find, I would urge you to find a couple more before you start hiding. By finding caches first, you get the experience of seeing good and bad hiding places in action and you'll know what is a waterproof container and what isn't.

To answer your question:

A cache should contain an info sheet for non-cachers that refers to geocaching.com so they know what your cache is for. A log can be pretty much a notepad, but it's a good idea to note some important details like the GC-code, your profile name and the hide date in there.

What is more important is to label the outside of your geocache whenever possible, so it's recognizable as a non-treatening container.

Well, the "Official Geocache" stickers that Groundspeak sells have spaces for the cache name, the waypoint (the coordinates, I assume), the GC code, who hid the cache, and contact info.

My log books and log sheets include an edited version of Groundspeak's cache note text (see the links in the right column on the Hide and Seek A Geocache page), the name and GC code of the cache, my geocaching handle (niraD), and my phone number.

The geocaching policy of one of the local parks requires that caches be labeled as geocaches, that they contain "a note describing the activity to an unintentional finder" (i.e., a cache note) and the owner's contact info. Including the name and GC code of the cache helps "unintentional finders" to identify the correct listing if they are interested in finding out more about geocaching; it may also help geocachers on occasion.
