What’s the best way to have multiple caches to make out a shape on the map?
 

For example a smiley or text with geocaches. Do you do need to pin it by hand or is there a better method to do this?

You need to place your "art" on top of a map, collect the coords for each point (separated 161m at least) and then assign geocaches for them, usually Mysteries, Multis or Letterboxes because they can be on any other location than the one shown on the map.

Several prior discussions of this question recommend Google Earth and its overlay image function.  For GeoArt designs that aren't overly complex, there is also this friendly online tool made by a trusted geocacher, Hyliston.
