Unable to complete the Title page of a new Adventure Lab

After entering the title and description of an Adventure Lab that I'm creating I choose a jpeg photo, identify the theme, put in the coordinates, and check the box indicating I've read the terms.   I then click continue and instead of going to the next page, it brings me back to what I've just entered, only the photo has disappeared as well as the theme I had identified.  An error message appears at the bottom of the page telling me that I need to identify at least one theme.  Any fixes to this?
