Error 500 on PQ Submission

I have gotten this error maybe 10 times in the past 5 years over 100's of PQs.  A re-submit a second later works and the error does not reappear; however for the past 3 days, I get it EVERY time I try to generate a PQ.  I tried it many times on two Macs on both Safari and Chrome and cannot generate a single PQ.  

Any assistance would be appreciated.  Thank you
