The official guidelines state 2 miles, but if you get permission from your local reviewer you can do more.

Not sure about that. But I know for sure that there is some older mystery caches that was hidden before a maximal distance was mandatory. These caches' final waypoint and posted coordinates are more than 2 miles/3.2 kilometers far away. By reading logs you can see a lot of shocked reactions written by geocachers unaware of this "souvenir of the past".

Exceptions in our area are pretty much non existent.   A handful of older Puzzles might be found out there, but the number of exceptions for various geoart projects made that portion of the Guideline pretty pointless, to the point that HQ felt a need to reign it in a bit.

Just wondering if there are any rules for the actual distance from the bogus coordinates for a puzzle/mystery cache?

So ultimately, it comes down to why you're asking.

If you're hiding a new puzzle cache, then the 2mi/3.2km limit is pretty firm. Exceptions have been made in the past, but you're unlikely to get one now.

If you're working on existing puzzle caches, then there will be exceptions. The older the cache, the more likely that it might have been granted an exception, or that it might have been created before the 2mi/3.2km guideline was enforced.

Finally, there is no limit to the distance between the first stage and the final stage of a multi-cache. There are some multi-caches that require traveling significant distances, and a few where the first stage and the final stage are on different continents.
