If its the official Geocaching app from Groundspeak, while actively navigating to a cache (map screen view), in the upper right hand corner, tap the button with the 3 dots on it (in older versions of the app it's a flag next to the Compass button). You can enter a new waypoint from there. It will drop a flag on the map at the new coordinates. Tap the flag and select "Set As Target" This question gets asked all the time, it's not real obvious.
