It's a scale of 1 to 5 in 0.5 steps. i.e. 1, 1.5, 2, 2.5, 3, 3.5, 4, 4.5 & 5 1 is easiest to find or get to, 5 is hardest. Actually 1 terrain means that a wheelchair can get to it, 5 terain means you need scuba gear, rappelling gear, helicopter or other special equipment. But sadly as these ratings are selected by the cache owner, and can be changed at any time after a reviewer publishes a cache, they very nearly are not a useful system. As for difficulty, there's discussions on puzzle caches having another rating, how hard to solve the puzzle, but as it is, difficulty means how hard it is to find the cache, a coleman cooler in the open would be a 1, a nano drilled into a fist sized rock among thousands of exactly the same rocks would be a 5. But again, sadly, some people think something they hide is much harder than it is. Us well seasoned cachers can find almost anything. But at least here the rating means a little more than nothing.
